How to File and Publish your FBN

Follow These Simple Steps
|

1.)
|
Complete the Fictitious Business Name Statement form. Easy-to-follow instructions are included with the form.
|
|
|
|
|
|
|
|
|
2.)
|
Print and sign the form in Blue Ink. The County will not accept the form without an
original signature.
|
|
|
|
3.)
|
The fee for publishing once a week for four weeks is $75.00. The County filing fee $30.00 for one business name and one owner name. Each addition business name or
owner name is $7.00

|
|
4.)
|
Mail the completed and signed form to:

If your business is a corporation, you must also send a copy of the Articles of
Incorporation, or if a LLC, the Limited Liability Papers. If Corporation or LLC also print title of the officer.

Please include your daytime phone number along with an email address.
We do the rest:
As soon as we receive your completed form, we will file your form with the Contra Costa
County Clerks office. Your copies including a certified copy will be returned to you after
the filing is completed through the mail.
We will then call you for your credit card information. We accept all credit cards except
American Express.
We will publish your FBN in our paper the Contra Costa News Register once a week
for four weeks, which is required by the County. After typesetting your FBN we will
send you a clipping by email for you to review, making sure all the spelling is correct.
(Please note, that everything on the form has to be published as written and filed no changes are allowed once filed).
After the last date of publication we will file a proof of publication with the County and mail you a filed copy for your records. The process is complete when you receive your copy of the filed proof of publication.
