I lost my receipt/DBA form. How can I get a new one?
If you need to print out a new receipt or DBA form, simply click on the Services button on the left side of this page and enter your Transaction ID and your email in the appropriate section and hit the submit button. Your receipt page will show up and from there you can generate a new DBA form.
I lost/forgot my Transaction ID. How can I find out what it is again?
If you need to find out what your Transaction ID is again, click on the Services button on the left side of this page and enter your email in the appropriate section and hit the submit button. An email with your Transaction ID will be sent to your email address shortly.
Okay, I printed out my DBA form and am still confused, what exactly do I need to mail in?
If you have successfully printed out your DBA form all you need to do is sign the form in black ink and mail it in along with your payment (if you chose not to pay with your credit card) and a copy of your articles of incorporation (if you are a LLC or a corporation). Its that simple.
I don't feel comfortable paying online with my credit card. Can I pay some other way?
Our entire website is secured using SSL technology which is certified daily by Thawte. We are also certified daily by McAfee. As well, we do not store your credit card number and information so your privacy and security are guaranteed on our side. However, if you still feel uncomfortable using your credit card, we do allow you to choose Paper Check as a payment option. If you choose to pay this way then you will need to include a check, money order or cashier's check when you mail in your signed DBA form.
